Job Title: Medical Social Worker Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 1 year acute care experience required License: California state license required Certifications: BLS, LCSW, MSW Must-Have: - 1 year of acute care hospital experience required - Experience with case management and discharge planning - Knowledge of charting systems, specifically Epic - Ability to work with adults and geriatrics Description: This position requires the provision of social work services in a medical setting. The role involves assessment, coordination, and planning for discharge processes. Social workers will manage caseloads of 20-23 patients. Collaborative work with medical teams, insurance case managers, and post-acute care providers is essential. The position supports the care management department, ensuring timely and effective discharges. Candidates must be able to navigate diverse patient needs and facilitate care plans tailored to individual circumstances. Weekend work may be required, with scheduling dependent on departmental operations. The social worker will perform initial evaluations within 24 hours of patient admission. Familiarity with community resources and patient advocacy is highly valued. Strong communication and crisis intervention skills are necessary for this role. Attention to patient needs and adherence to ethical standards are critical components of the position.
